You can expect to be at our office for approximately 1 hour for your first visit. This includes time to fill out paperwork and collect your health information. Please bring a photo ID and insurance card (if applicable) to your first appointment. If you would like to expedite the process you are welcome to print and fill out the initial paperwork below. If you have a question for the chiropractor prior to coming in please submit it under the "Contact Us" tab.
We welcome both out-of-pocket and insurance patients. We accept most major insurance plans.
